1. What disease is Ultracet used to treat?

Ultracet is indicated for the management of severe acute pain when other alternative treatments have not been effective.

2. Some notes when using Ultracet

  • Principle: Use the lowest effective dose for the shortest time suitable for individual patient treatment goals, taking  care not to use the drug for more than 5 days.
  • Children > 12 years old, adults: Start treatment with 2 tablets every 4-6 hours as needed for acute pain relief,  up to 8 tablets/day .
  • Each patient's dose of medication will be prescribed by the doctor differently based on the severity of the pain, the patient's response, and risk factors for addiction and drug abuse. Therefore, absolutely do not arbitrarily take drugs prescribed by others.
  • Patients with  severe renal  impairment (creatinine clearance less than 30 ml/min): Do not exceed 2 tablets every 12 hours.
  • Do not use concurrently with other medicines containing paracetamol (Panadol, Efferalgan) or tramadol.

3. In which cases is it absolutely forbidden to use Ultracet?

  • Children under 12 years old.
  • Postoperative treatment in children under 18 years of age.
  • Patients with tonsillectomy or other types of surgical removal.
  • Significant respiratory failure.
  • Uncontrolled acute or severe asthma or lack of resuscitation equipment.
  • Suspected gastrointestinal obstruction.
  • Hypersensitivity to tramadol hydrochloride, paracetamol or any of the ingredients.
  • Are using monoamine oxidase inhibitors (MAOIs) or have used MAOIs (medicines used to treat depression) within the past 14 days.

4. What precautions should I take when taking Ultracet?

  • Risk of convulsions.
  • Risk of suicide: Do not prescribe to patients at risk of suicide or prone to drug addiction.
  • Patients with adrenal insufficiency should be very cautious when using Ultracet .
  • For patients with chronic respiratory failure: Monitor closely, especially when starting the drug and changing the dose.
  • For patients with severe  hypotension  : Monitor drug initiation and change dose. Avoid using Ultracet in patients with circulatory shock.
  • For patients with increased intracranial pressure, brain tumor, head trauma or impaired consciousness: monitor the effectiveness of analgesia and respiratory failure. Avoid using Ultracet in patients with impaired consciousness  or coma.

5. What are the side effects of the drug?

5.1. Common unwanted effects

  • Gastrointestinal: constipation, diarrhea, nausea.
  • Loss of appetite, dizziness and sweating.
  • Sleepy.

5.2. Serious but uncommon side effects

  • Addiction or abuse of drugs.
  • Life-threatening respiratory failure.
  • Neonatal withdrawal syndrome: Occurs when pregnant women take this drug during pregnancy. At birth, the baby is still dependent on the drug. Babies are born with symptoms of poor feeding, rapid breathing, tremors and fussiness.
  • Liver toxicity.
  • Epileptic.
  • Serotonin syndrome: appearance of a group of signs such as involuntary myoclonic tremor, agitation or sweating, uncontrolled nystagmus, tremor and hyperreflexia, increased muscle tone, temperature rise of 38°C and vibration eyeball twitch.
  • Suicidal.
  • Adrenal insufficiency.
  • Severe hypotension.
  • Hypersensitivity reactions.
  • Opioid withdrawal syndrome.

Inform your doctor immediately when you experience any unwanted effects of the drug.

6. What drugs can Ultracet interact with?

Use caution if you are taking any of the following medicines:

  • CYP2D6 inhibitors: quinidine, fluoxetine, paroxetine and bupropion…
  • CYP3A4 inhibitors: macrolide antibiotics, antifungal drugs…
  • CYP3A4 inducers: rifampin, carbamazepine, phenytoin…
  • Painkillers acting on the central nervous system: morphine, codeine...
  • Serotonin inhibitors: antidepressants.
  • MAO inhibitors: phenelzine, tranylcypromine, linezolid...
  • Muscle relaxants: depress neuromuscular and cause respiratory depression.
  • Diuretics: reduce the diuretic effect.
  • Digoxin: one of the drugs used to treat heart failure.
  • Warfarin: one of the anticoagulants.
  • Other opioid analgesics (nalbuphin, buprenorphine, pentazocin): reduce analgesia and the risk of withdrawal syndrome.

7. Can pregnant and lactating women use Ultracet?

For pregnant women: Taking Ultracet may harm an unborn baby, causing withdrawal syndrome in the newborn baby (as mentioned in the Adverse Effects section above).

Lactation: Ultracet is not recommended for use before obstetric surgery or postpartum in nursing mothers because safety in the infant has not been studied.

8. Symptoms and treatment of overdose

  • Respiratory depression.
  • Convulsion.
  • Loss of appetite, nausea, vomiting.
  • Uncomfortable pale sweating.
