Galvus Met is a drug used to treat type 2 diabetes . Both active ingredients, vildagliptin and metformin, help control blood sugar levels.

  • Vildagliptin: belongs to a new generation of antidiabetic drugs, improves insulin secretion by the pancreas dependent on blood glucose levels. 
  • Metformin: works by helping the body use insulin better and does not cause excessive hypoglycemia. In addition, Metformin also exhibits beneficial effects on lipid metabolism (reducing total cholesterol, LDL-cholesterol and triglycerides) and preventing complications of diabetes.

What are the uses of Galvus Met (metformin/vildagliptin)?

  • Treatment of type 2 diabetes mellitus: in adult patients who are unable to achieve complete glycemic control with maximally tolerated doses of metformin or have been treated with the combination of vildagliptin and metformin as separate tablets.
  • In combination with a sulphonylurea (that is, triple combination therapy): as an adjunct to diet and exercise in adult patients partially controlled with metformin and a sulphonylurea.

Contraindications

Hypersensitivity to the active substance or to any of the excipients.

Contraindicated with objects: 

  • Severe renal failure (GFR <30 ml="">
  • Diabetic pre-coma.
  • Acute metabolic acidosis (eg, lactic acidosis, diabetic ketoacidosis).

Acute cases have the potential to change kidney function, such as:

  • Loss of water.

Dosage of Galvus Met (metformin/vildagliptin) 

The dose of Galvus Met (metformin/vildagliptin) depends on your medical condition and tolerability. However, you should not take more than 100 mg of vildagliptin in a day. Cases with dose differences such as:

Not well controlled with the maximum tolerated dose of metformin monotherapy

  • The starting dose of Galvus Met can be 50 mg/500 mg or 50 mg/850 mg or 50 mg/1000 mg twice daily (total vildagliptin dose is 100 mg daily) depending on the dose of metformin used.

Dosage for other cases:

  • Switching from concomitant use of vildagliptin and metformin as separate tablets
  • Galvus Met should be started with the dose of vildagliptin and metformin already being used.
  • Not well controlled in dual combination with metformin and sulphonylurea

Take Galvus Met 50 mg twice daily (total vildagliptin dose is 100 mg daily) and the dose of metformin is similar to that already used. When Galvus Met is used in combination with a sulphonylurea, a reduction in the dose of the sulphonylurea should be considered to reduce the risk of excessive hypoglycemia.

How to take Galvus Met (metformin/vildagliptin)  

Swallow the tablet whole with water. The drug should be taken with food to reduce the gastrointestinal side effects of metformin.

In particular, if you are on a diabetic weight control diet, continue to exercise and eat right even while taking Galvus Met.

Some very rare patients may have symptoms such as: sore throat, runny nose, fever; lactic acidosis (drowsiness or dizziness, severe nausea or vomiting, stomach pain, irregular heartbeat or deep, rapid breathing); skin redness, itching; vitamin B12 deficiency (pallor, fatigue, psychiatric symptoms such as confusion or memory disturbances).

When combined with a sulphonylurea, patients may experience problems such as: dizziness, tremor, weakness, excessive hypoglycemia, excessive sweating. You should stop taking Galvus Met and see your doctor right away if you experience the following side effects:

  • Angioedema.
  • Lactic acidosis. 
  • Liver disease (hepatitis). 

Drug interactions  

Vildagliptin has no clinical evidence of drug interactions with other oral antidiabetic agents (glibenclamide, pioglitazone, metformin hydrochloride) and some drugs such as amlodipine , digoxin, ramipril, simvastatin, valsartan or warfarin after administration. in combination with vildagliptin. 

Metformin has interactions with drugs containing active ingredients such as furosemide, nifedipine, glyburide, amiloride, digoxin, morphine, procainamide, quinidine, quinine, ranitidine, triamteren, trimethoprim or vancomycin...

Some drugs have a tendency to increase blood sugar such as thiazide diuretics, corticosteroids, phenothiazines, phenytoin, nicotinic acid, estrogens, oral contraceptives, calcium channel blockers and izoniazid. Blood glucose levels should be closely monitored and the dose of metformin should be adjusted when administering or discontinuing these agents. 

What to do in case of overdose?
